Amy McAllister, 26, of Conway was arrested Thursday afternoon and charged with Forgery, Assisting in the Preparation of a False Income Tax Return and two counts of Conspiracy, according to the State Department of Revenue.
The department says she gave a fraudulent W2 form to a tax preparer in order to have 2011 income taxes prepared even though she did not work for the employer listed on the tax form. The forged form stated McAllister earned $13,200 in wages, and had $594 withheld and claimed federal and state refunds of $5,740 which she was not entitled to, according to the South Carolina Department of Revenue.
Investigators say she conspired to willfully assist in the preparation of a fraudulent tax return with others.
