Jermaine Mouzon, 31, was arrested Monday, by state department of revenue officials, for one count of assisting in the Preparation of a False Income Tax Return, one count of Forgery and three counts of Conspiracy.
Mouzon, of Myrtle Beach, is accused of presenting a fraudulent W2 form to a tax preparer in order to have a 2011 income tax return submitted in his name.
Mouzon did not work for the employer, according to department of revenue officials, but said he earned $14,897 in wages and had $670 in state taxes withheld. He claimed a federal and state tax refund totaling $8,684 to which he was not entitled, officials said.
If convicted of the charges, Jermaine Mouzon could face up to $15,500 in fines and/or 25 years in prison.
